New Student Enrollments

Documentation Needed:

Proof of Age Address (Includes, but is not limited to, any ONE of the documents listed below)

·   Child’s birth certificate: Mandatory

·   Child’s baptismal record

·   Passport

·   Court documents

 Proof of Address (Includes, but is not limited to, any TWO of the documents listed below)

·   Current utility bill

·   Illinois driver’s license or State of Illinois identification card

·   Deed

·   Employee identification number

·   MediPlan/Medicaid card

·   Court documents

·   Illinois Department of Public Aid card

·   Stamped United States Post Office change of address form

·   Illinois state aid check/social security check

Health Requirements (Needed at the time of enrollment for new students)

Children are encouraged to submit their required immunizations and physicals before the start of the school year and no later than October 15 of the current school or they will face expulsion from school.

  • Physical Examination Requirements

All students must have a physical examination dated within one year of: Entering schools in the State of Illinois for the first time, at any grade level

Entering preschool, up to age six (physical exam and lead screenings)

Immunization Requirements

·   Diphtheria, Pertussis (Whooping Cough), Tetanus (DTP/Td)

·   Inactivated Polio

·   Measles

·   Rubella

·   Mumps

·   Hepatitis B

·   Varicella (Chicken Pox)

·   Haemophilus Influenza, Type B (HIB)

  • Dental Requirements (Required for ALL NEW STUDENTS)

(Pre-K- proof of a booked appointment will do) All Pre-K, kindergarten, second, and sixth-grade students must have a dental exam completed by a licensed dentist before May 15th of the current school year.

Vision Examination: Due upon enrollment or no later than 10/15/25 for:  Entering the State of Illinois for the first time at any grade level, entering kindergarten.

Returning Students

6 forms need to be returned either via email to the clerks or during the first week of school. Some have already returned them in the last week of school. (I will also be sending this out starting in July via Aspen)
